How it works
Cagrilintide is a long-acting synthetic analog of amylin, a hormone the pancreas releases alongside insulin to help regulate satiety and post-meal metabolism. Researchers modified native amylin's structure to resist the fibril formation that limits its natural stability, extending its duration of action enough for once-weekly dosing. Cagrilintide has been studied on its own, but it is best known as the amylin component of CagriSema, an investigational once-weekly combination with the GLP-1 receptor agonist semaglutide, both under development by Novo Nordisk for chronic weight management.
Benefits & side effects
The dominant safety signal across CagriSema trials has been gastrointestinal: nausea, constipation, and vomiting occurred in roughly 73-80% of CagriSema-treated participants across the two REDEFINE trials, versus about 34-40% on placebo, mostly described as transient and mild to moderate.[9][10] Because CagriSema combines cagrilintide with semaglutide, much of its established warning profile comes from semaglutide's own labeling rather than cagrilintide-specific data, including:
- Acute pancreatitis and gallbladder disease
- Hypoglycemia risk when used with insulin or other glucose-lowering medicines
- Kidney injury related to dehydration from vomiting or diarrhea
- Increased heart rate and, in people with type 2 diabetes, diabetic retinopathy complications
Cagrilintide-specific long-term safety, pregnancy, and drug-interaction data remain limited in the absence of a standalone approved label.[14]
