How It Works

HMG works by supplying exogenous FSH and LH activity to reproductive tissue under medical supervision. FSH supports follicle growth in the ovaries and Sertoli-cell function (which supports sperm production) in the testes, while LH-like activity drives steroid hormone production and, in men, testosterone output from Leydig cells.[1] Human chorionic gonadotropin (hCG) activates that same LH receptor pathway, which is why it is often paired with HMG to trigger final egg maturation in fertility cycles.

Having a receptor pathway to act through does not guarantee a result: whether follicles mature, ovulation occurs, or sperm counts improve depends on the underlying diagnosis, hormone levels, and clinician monitoring.

What the Research Shows

The strongest evidence for HMG sits in reproductive medicine: ovarian stimulation protocols in IVF and other assisted reproductive technology (ART) cycles, and ovulation induction in select anovulatory conditions such as polycystic ovary syndrome unresponsive to first-line treatment. Systematic reviews have compared urinary gonadotropins like HMG against recombinant FSH in ART as a treatment-category comparison, not as a declaration that one is superior for all patients.

In men, gonadotropin therapy combining hCG with an FSH-containing product such as HMG has a defined, evidence-supported role in hypogonadotropic hypogonadism, where the pituitary fails to adequately signal the testes. Evidence for broader "idiopathic" male infertility is weaker, and no credible clinical evidence supports HMG for weight loss, anti-aging, or general wellness — none of which appear anywhere in its approved labeling.

Safety & Side Effects

Commonly reported effects include headache, abdominal pain or bloating, nausea, injection-site reactions, and mood-related symptoms during treatment. More serious risks described in menotropins labeling include:

  • Ovarian hyperstimulation syndrome (OHSS): enlarged ovaries, fluid shifts, and in severe cases thromboembolic or respiratory complications
  • Multiple pregnancy from multifollicular development
  • Ovarian torsion
  • Ectopic pregnancy and spontaneous abortion
  • Possible association with ovarian neoplasms after repeated stimulation, though causality is difficult to establish

Contraindications include hypersensitivity to menotropins, elevated FSH suggesting primary ovarian failure, uncontrolled endocrine disorders, sex hormone-dependent tumors, pituitary or hypothalamic tumors, unexplained abnormal uterine bleeding, non-PCOS ovarian cysts, and pregnancy.[1] These risks are why fertility treatment with HMG is medically monitored rather than self-directed.

Regulatory Status

HMG has a genuine, narrow FDA approval: MENOPUR (menotropins) is approved specifically for the development of multiple follicles and pregnancy in ovulatory women undergoing an ART cycle.[1] This is one of the few compounds on this site with an actual approved medical indication rather than purely investigational status.

That approval does not extend to every use associated with the name "HMG." Off-label uses such as broader ovulation induction outside the labeled population, male infertility treatment, or any weight-loss, anti-aging, or bodybuilding application are not FDA-approved uses of menotropins. Compounded, imported, or vendor-marketed "HMG peptide" products sold outside regulated pharmacy channels are not FDA-reviewed for safety, effectiveness, or quality, and should not be assumed equivalent to an approved prescription product.
